The subscription maximizes Dr. Blake’s role as a comprehensivist (caring for you directly) and as a coordinator of care (keeping your care team on the same page).

- Unlimited Visits – all new patient and follow-up appointments
- Comprehensive Labs – strep, flu, and COVID tests; pregnancy tests; urinalysis; stool blood tests
- In-Office Procedures – wart or actinic keratosis freeze, skin tag removal, skin biopsy, simple incisions, laceration repair, ear wax removal, suture removal, spirometry, and EKG.

Because Peace of Mind is Priceless
While each piece of our Direct Primary Care (DPC) clinic is a benefit to you, we think that when it is put together it really shines.
With your subscription, you can contact Dr. Blake directly. So when you feel ill, you can text him, call him, or reach out to him online. Your subscription includes in-office time, but it also covers any care Dr. Blake provides without a scheduled visit. If he can’t resolve your concern via phone/text, then he can arrange to see you that day (or the next day, if after hours).
Your annual subscription includes unlimited preventative visits. You are not limited to the one “free” visit that your insurance pays for. Your health is what matters, not shoehorning your concerns into what your insurance considers a well visit.
Let’s say your clinic visit shows your ailment to be strep throat. We can then dispense the amoxicillin to you during the visit. And all of this for the ease of a monthly subscription price

Please Note: A subscription to Blake Family Medicine is not an insurance product. We encourage all of our patients to keep their own insurance for major events. However, given that your primary care needs are covered by your subscription, you may prefer an insurance plan that has lower monthly premiums as it doesn't need to cover your primary care visits.
